This page is the secure container. Drop your Google Form's embed link in and it appears right here — patients never leave Care4, and their data goes straight into your BAA Workspace.

1Open your intake form → Send → the < > embed tab.
2Copy the src link (ends in /viewform?embedded=true).
3Paste it into FORM_URL near the bottom of this file.
4On July 22, swap that one link for the BAA Workspace form. That's the whole compliance switch.
